The Quality Alliance Patient Safety Organization (QAPSO), an Illinois limited liability company, is listed as a federally-recognized patient safety organization (PSO). It is a component of the Midwest Healthcare Quality Alliance; Memorial Health System; and Southern Illinois University HealthCare. 

Patient Safety Organizations were created as a result of a federal law, The Patient Safety and Quality Improvement Act of 2005, with the general purpose of encouraging voluntary adverse event reporting, and allows healthcare providers to collaborate and share information on patient safety and quality of care for broader improvements nationwide. 

In exchange for participating in patient safety organization activities, healthcare providers are afforded federal level confidentiality protection for this data and information. The MHQA seeks to utilize these benefits for collaborating with our regional partners on patient safety, healthcare quality, and population health improvements.
