Todd S. Roberts, MBA, CLSSMBB, FACHE serves as the Vice President of Quality and Safety and Chief Quality Officer for Memorial Health System in Springfield, Illinois. In his role with Memorial, he oversees Patient and Environmental Safety and Accreditation, Operations Improvement, Decision Sciences, and the Enterprise Project Management Office. Under Todd’s leadership, Memorial Health System has won numerous state, national, and international quality awards and honors, including the 2016 American Hospital Association McKesson Quest for Quality Prize, the International Hospital Federation’s top honors for innovations at the 2015 IHF World Congress, and four Illinois Quality Excellence Achievement Awards. He is a is a Certified Lean Six Sigma Master Black Belt and a Fellow of the American College of Healthcare Executives.

Carol Hafley, MHA, RN, FACHE, CPPS, System Director, Quality Alliance Patient Safety Organization
Carol Hafley holds a master’s degree in health administration and a Bachelor of Science in Nursing from the University of Missouri-Columbia. She most recently worked as the assistant director for the Center for Patient Safety in Jefferson City, Missouri, where she established and directed the patient safety organization business line for emergency medical services, in addition to leading numerous statewide patient safety projects. She is a Fellow of the American College of Healthcare Executives, a Certified Professional in Patient Safety and a certified lean six sigma green belt. Ms. Hafley’s previous clinical experience includes medical/surgical nursing, coronary intensive care, and home health. Her administrative background includes utilization and quality review, managed care, case management, coding and clinical documentation improvement.
